Palliative Care RN who will provide disease specific education to patients/families to include end-of-life conversations. Visits will occur in the pt's home or facility to monitor pt needs, follow-up on symptoms following medication changes, offer navigational support, and provide communication to the IDT to include internal/external providers and care team members.
Responsible for education to patient and family related to their specific disease processes to include conversations related to end-of-life. Will provide home or facility visits for patients to follow-up on medication adjustments/reconcillation, and evaluation of symptoms. Willl have open communication with Palliative Care providers, PCP's, or other providers involved on the patient's team related to the pt's condition and needs. Will act as a nurse navigator as needed.
*Graduate of approved school of nursing with current North Carolina license or RN multi-state licensure privilege through the Nursing Licensure Compact Requires BSN; minimum of two years previous experience in nursing to include acute care, med-surgical or home care nursing. Palliative Care or end-of-life experience preferred.
*Possess valid North Carolina Driver's License.
*National Certification (CHPN) recommended after completion of at least two years of full-time as a RN in palliative nursing practice.
*CPR required.
